Middle School Clinic Payment

 I, the undersigned, individually and as a parent and/or guardian of (player’s name), a minor, ask that she be admitted to participate in the sports clinic sponsored by North Hunterdon Jr Lions Volleyball, Inc. I do hereby agree to release, discharge and hold harmless the NHJLV and the North Hunterdon-Voorhees High School District and their coaches, clinicians, employees and volunteers from causes, liabilities, damages, claims and demands whatsoever on account of any injury or accident involving the said minor arising out of the minor’s attendance at the sports clinic or in the course of competition and/or activities held in connection with the NHJLV sports clinic. I certify that my applicant is in good physical condition and has my approval to participate in in this clinic.

I hereby agree that my daughter, may be filmed, videotaped and photographed, and have her image only, (name will not be used) for advertisement, public relation, promotion or any other non-profit purpose(s) by North Hunterdon Jr Lions Volleyball.  I agree to waive any present or future claim for compensation or consideration.  I also waive my right to inspect or approve the finished product, including any photos that may be posted online.
